Samuel Lee's Last Day is a VHS Tape recording found within Chapter 3: "Deep Sleep" of Poppy Playtime. The tape can be obtained in one of the rooms from the cupboard found inside Home Sweet Home. The tape records Ms. Brooks and the children in the Playcare celebrating Samuel Lee's adoption, the children being unaware of what is actually going to happen to Sam. During the tape a toddler, likely Samuel, stares at the camera, looking like he’s about to cry.

The tape begins with the noises of children celebrating an adoption in Playcare. Ms. Brooks tells Dr. White that they would not miss an adoption. Two children ask Ms. Brooks who is going to get adopted, shushing them after. Ms. Brooks tells the other children that Samuel Lee is going to get adopted, to the cheers of the crowd. Ms. Brooks tells them to say one last goodbye. The tape ends with all the children altogether saying one last goodbye to Samuel Lee and applauding as he leaves.

(celebrating in the background) Dr. White: "Ah, and here they all are." Ms. Brooks: "Well of course, they'd never miss this!" Child 1: "Who is it, Ms. Brooks? Who's going away?" Child 2: "Tell us! Tell us!" Ms. Brooks: (shushes) "This week, Dr. White here has selected our very own Samuel Lee!" (screams) Ms. Brooks: "Now, before he goes, let's all give Sam one last goodbye, shall we?
