Note 4 is a document found in Chapter 3: "Deep Sleep" of Poppy Playtime. It serves no purpose in completing the game, however, it gives deeper insight into Miss Delight and the fate of the other teachers in the weeks following The Hour of Joy.

In the School, in one of the side rooms a toy of Bunzo Bunny can be seen hanging by its neck out of one of the ceiling vents, and Note 4 is attached to him.

- This provides an explanation for why Miss Delight stops moving whenever The Player looks at her.
- The frequent changes from normal print to all capital letters seem to emphasize Miss Delight's starvation-fueled descent into madness.
- Similar to Note 2, and Note 3, this one also seems to have a bloodstain on its bottom right corner, and is oddly shaped like a human.
