The Chapter 4 Cinematic Trailer: The Doctor is a cinematic trailer for Poppy Playtime's fourth chapter, in the form of a recording. It was uploaded onto Mob Entertainment's YouTube channel.

Its purpose was to reveal the main antagonist of the chapter: The Doctor, a character who had been getting teased for over 2 years at the time of the trailer's release.

The video begins with a photosensitivity warning before the Mob Entertainment logo appears on screen. However, the footage begins to glitch - with the words "REBOOTING" and "WAKE UP" flashing repeatedly on the screen.

It then cuts to the perspective of a camera inside a greenish-lit room aimed at a tiled floor, as Harley Sawyer's deep distorted voice says "Where am I?". Several static images of several of the experiments, including Boxy Boo, The Prototype, Mommy Long Legs, Yarnaby, Huggy Wuggy and CatNap flash into view. The words "BIGGER BODIES" and "MY CREATIONS" are also visible. The camera then slowly starts to move upwards as Sawyer says with confusion "What is this?".

More flashing images of what look like rooms in a laboratory and the words "NO BODY" and "I AM BROKEN" appear with Sawyer saying "Wrong. This is wrong". The camera continues to move up until a round metal platform comes into view. More images of several strange locations flash into view along with the words "I AM DIFFERENT" and "SOMEWHERE", as Sawyer voices his curiosity. He then threatens Leith Pierre and Bruno White, claiming that they have made a mistake as some sort of machine is seen powering up along with the words "NOWHERE AND YET EVERYWHERE". The moving camera then reveals a darkened figure with a square-shaped head standing on the metal platform with many static TV monitors switching on around him, all the while a bell can be heard tolling.

As one last message: "I AM NO MAN" appears, the figure's head is shown to be a TV as the image of a purple eye suddenly appears on its screen. Sawyer claims how it was thought that his mind could be broken like many others, but that "[they] were wrong". The video then shows the chapter’s name: "Safe Haven" along with the release date of 01/30/25.

W-w-what is this?.." (A functional security camera is heard moving, before switching to a different feed.) The Doctor: "W-wrong..." (Footage is heard fast-forwarding.)
